KEY COMPONENTS
Our comprehensive program consists of key elements such as needs assessment, life skills planning, targeted case management, concrete support. education in self-sufficiency, linkages to community resources, suicide prevention education, social change, and equity.
QUALIFICATIONS FOR SERVICES
Transitional Age Youth ages 16-26 must meet ONE or more of the following criteria:
- Victim of a crime
- At-Risk for homelessness
- Substance abuse history
- Mental health history
- Survivor of childhood trauma
- Long term institutional care
- Juvenile justice system history
- Aging out of DCFS*
- Survivor of domestic violence

PROGRAM SERVICES
The Pathways to Employment program aims to improve self-esteem and autonomy. The evidence based independent living skills curriculum assists in preparing participants with the transition into adulthood with a self-determined plan.
MENTAL HEALTH
Compassionate mental health supportive services are provided for teens, including counseling and coping strategies. These services are offered in a safe and non-judgmental environment, empowering teens to navigate challenges and thrive.
LIFE SKILLS WORKSHOPS
This program includes weekly workshop meetings aimed at supporting Transitional Aged Youth (TAY) in pursuing their educational and employment goals. Skills aid participants in transitioning into adulthood with a self-determined plan.
TARGETED CASE MANAGEMENT
Assistance is provided in obtaining essential identification (e.g., SSN, birth certificate, and driver’s license). Additionally, we offer help in applying for benefits and provide personalized employment planning for job readiness.
RESOURCES AND LINKAGES
We provide a wide array of resources and linkages for teens including: needs assessment, targeted case management, community resource connections, concrete support, social change initiatives, and suicide prevention education.
